Launch of Consensus on the Primary and Secondary Care Interface

(Update 25 June 2022)

Cheshire and Merseyside has launched a new ‘consensus’ document containing a set of clinically led principles to guide reviews of pathways which have a common architecture of good quality and patient-centred communication.

The core purpose of this publication is to ensure we (the System) optimise access to the right care, giving our patients the very best outcomes. To enable this, we have set out a number of guiding principles which we should all commit to when interacting with colleagues. Abiding by these principles will encourage us to keep the patient at the centre of our decision making and ensure that actions taken are completed in a timely way, by the most appropriate individual or team and understood by all.
