The Remote Monitoring Programme has been established to extend and integrate the use of remote monitoring across Cheshire and Merseyside with the aim of developing a patient-centred regional digital architecture for self-care and monitoring, aligning all current areas of work into an integrated approach and platform for delivery.
The Remote Monitoring Programme consists of the following projects…
Remote monitoring was deployed rapidly across Cheshire and Merseyside to support patients with COVID-19 from August 2020. Eight ‘places’ have successfully implemented digital technology to support the national initiatives of COVID Oximetry @ Home and COVID Virtual Ward Service. To date over 4,000 COVID patients have been supported using digital technology.
As the prevalence of COVID reduces across the region attentions are now being focused on using remote monitoring to support virtual wards, long-term condition management, care homes and mental health.
